Job Description
A highly respected Home Care agency is seeking a Registered Nurse (RN) with exceptional administrative and leadership skills to join their team in the Ventura area. This RN will be responsible for visiting patients in their homes, assesing patient needs, and updating care plans.
Duties:
* Develop and maintain patient care plans in accordance with agency policies and procedures.
* Conduct patient visits to ensure that care plans are being followed and updated as necessary.
* Communicate with patients, their families, and other healthcare professionals to coordinate care and ensure patient satisfaction.
* Conduct regular reviews of patient charts to ensure that all required documentation is accurate and up-to-date.
* Collaborate with other healthcare professionals to ensure that patients receive the highest level of care.
* Work with agency leadership to develop and implement quality improvement initiatives.
* Ensure that all patient information is kept confidential and in compliance with HIPAA regulations.
* Participate in agency meetings and trainings as required.
Requirements:
* Must be a Registered Nurse with an active California nursing license.
* Strong organizational and administrative skills with the ability to prioritize tasks and meet deadlines.
* Excellent communication skills with the ability to build rapport with patients and their families.
* Ability to travel throughout the Ventura and surrounding area as required.
* Strong understanding of HIPAA regulations and the ability to maintain patient confidentiality at all times.
